The Community Health Clinic (CHC) provides non-urgent, quality healthcare to low-income, uninsured families in Collin County. Our dedicated volunteer healthcare providers and community volunteers work alongside clinical staff to serve those who need a health home the most.
We are seeking a Family Medicine Nurse Practitioner with a strong foundation in primary care to help manage chronic conditions such as diabetes, hypertension, and asthma. In this role, you will work closely with patients from diverse backgrounds and collaborate with the clinic team to provide essential medical care.
What You’ll Do:
- Independently assess and document clinical problems.
- Perform and document assessments and patient histories.
- Order and interpret appropriate laboratory and diagnostic studies.
- Order appropriate medication and treatments and complete refill requests.
- Educate patients and their families about health issues, preventive care and healthy lifestyle choices
What Makes a Successful Volunteer?
- Valid and current Texas Registered Nurse (RN) license with recognition as a Nurse Practitioner in the state of Texas.
- Valid and current national board certification by AANC or AANP as a Nurse Practitioner.
- Ability to work with diverse populations and age groups.
- A passion to help the underserved and uninsured.
- Ability to work as an effective member of a dedicated team of volunteers and paid staff
Time Commitment
The clinic is open Monday-Friday 8:30am-4:30pm. Some evenings and Saturdays are available. Commitments accepted as little as one day per month based on your availability. 4, 6, or 8hrs. shifts available
